The chapter reviews a participatory evaluation of cancer prevention and care services in Costa Rica under the lens of CAE principles with the aim of identifying opportunities and challenges in their application. The research was conducted as a retrospective case of study, and the authors have designed and applied specific instruments to collect the data from the participants in the evaluation. Trainings, adaptation of instruments, and appropriate facilitation were key elements to create the necessary conditions to engage stakeholders in evaluation beyond involvement more classical in consideration as mere sources of data. The CAE principles are a valuable reference to guide and inform evaluation processes highlighting the importance of the context and the knowledge produced by the evaluators and stakeholders involved. However, it is necessary that the principles are followed by a set of practical guidelines about how to set its main ideas in every step of the evaluation process. The results of the review of the participatory evaluation in Costa Rica make a contribution in this way.
